Your Mission will be to bring clinical insights into product decisions, as we build safe, production-ready clinical AI
This role is a unique role for doctors that are passionate about technology and solving thorny problems. As our Founding Clinical Product Engineer, you will help architect and own clinical development in the DoctorOS and our AI systems. You will design, build, and ship product-critical clinical intelligence used every single day by doctors delivering patient care.
By joining us now, you will define our tech stack and decide what we need to scale. Our current tech stack is
Frontend: React 19 with TypeScript
Backend: AWS serverless — Lambda, API Gateway, DynamoDB, and S3
Authentication + Database: Supabase
Tooling: Playwright for testing, Tailwind for styling, Radix UI for components
You will work on:
- The entire Product/AI lifecycle: inference pipelines, retrieval, fine-tuning, evals, safety mitigations, and monitoring.
- Clinical-grade eval & safety infrastructure: hallucination detection, error monitoring, model governance, incident tooling and system-level guardrails.
- MLOps & infra: reproducible training pipelines, CI/CD for models, audit trails, versioning, and data lineage.
- Human-in-the-loop clinical validation: working directly with doctors and our clinical leads to define acceptance criteria and run prospective evaluations.
- End-to-end shipping: design → prototype → deploy → measure → iterate.
- Future team growth: help hire the next engineers and shape the technical culture from the ground up.
There is no partition between research, engineering, and product…… here you are all three.
Who You Are
You should be a doctor that cares deeply about modern technology. Beyond that, we don’t care about pedigree. We care about what you’ve built and how you think.
You thrive when:
- The stakes are high and the feedback loops are tight.
- The constraints are real (safe, compliant clinical environments).
- You own outcomes end-to-end.
- You’re responsible for systems that must work and must be trustworthy.
You’ll be a great fit if you have:
- Been a founder or founding engineer
- Experience with LLMs, model evaluation, monitoring, or MLOps.
- A track record of shipping ML or production systems in high-stakes environments.
- High agency and product intuition: you chase impact, not tickets.
- Strong judgment around risk, safety, and quality in real-world deployments.
- Experience working full-stack (infra ↔ data ↔ product).
Bonus if you:
- Have worked in regulated environments
- Built software/AI as a medical device
- Have shipped tools used by clinicians
- Have founded or been early at a startup

What Success Looks Like
Months 1- 3: Learn, Ship, Integrate
You will:
- Stand up our LLM inference & eval pipeline.
- Ship your first end-to-end feature into clinical workflows.
- Implement core monitoring/metrics for model behaviour and safety.
- Become embedded with doctors to understand real-world clinical needs.
- Own a meaningful slice of the roadmap.
Indicators of success:
- You’ve shipped code used by clinicians.
- You’re independently diagnosing, designing, and delivering improvements.
- You’ve raised the technical bar.
Months 3- 6: Lead, Architect, Optimise
You will:
- Own full verticals of the AI stack (eg. safety, evals, RAG).
- Make architectural decisions that shape DoctorOS.
- Run experiments that measurably improve clinical throughput or decision quality.
- Build internal tooling for reliability, safety, and reproducibility.
- Help interview your future teammates.
Indicators of success:
- You’ve made major AI systems more reliable, faster, or safer.
- You’re the go-to person for at least one domain (infra, safety, evals, retrieval, etc).
- You’re influencing product direction directly with founders + clinicians.
Months 6–12: Own Strategy, Build the Org
You will:
- Own multi-quarter AI roadmap areas with minimal supervision.
- Lead key initiatives in clinical AI safety and reliability.
- Architect next-gen capabilities in DoctorOS (structured reasoning, tool use, advanced monitoring, clinician-feedback loops).
- Mentor new hires and help shape engineering culture.
- Represent Asterix in technical conversations with customers and partners.
Indicators of success:
- You’ve designed systems that become core infrastructure for the company.
- You’re trusted with company-defining decisions.
- You’re shipping features that materially reduce clinician workload and improve care outcomes.
